Echocardiography For the Anesthesiologist

چکیده
use of 3-dimensional (3-D) echocardiography. Basic Examination Transesophageal echocardiography provides an excellent diagnostic and monitoring tool for anesthesiologists in the operating room. The TEE examination can be broken down into complete/ comprehensive and abbreviated forms; the user may select either depending on the urgency of the situation and other clinical responsibilities. As with any invasive procedure, the potential risks and benefits of TEE should be discussed with the patient and preoperative informed consent obtained. A study by researchers at Brigham and Women’s Hospital, in Boston, retrospectively reviewed more than 7,000 TEE examinations and found rates of procedure-related morbidity and mortality of 0.2% and 0%, respectively. Although rare, the most common TEE-related injuries were odynophagia, dental trauma, malpositioning of the endotracheal tube, upper gastrointestinal hemorrhage, and esophageal perforation. Few contraindications exist to insertion of the TEE probe; these include dysphagia, odynophagia, significant reflux, hematemesis, history of gastric and/or esophageal pathology (a hiatal hernia is not a contraindication but may complicate imaging), and significant resistance during insertion or advancement of the probe (Table 1). متن کاملEnlarged Right Atrium Falsely Interpreted as a Mediastinal Mass: A Case Report
Right atrial dilation due to left heart disease is a common complication among adults. The present review aimed to describe a case of massively dilated right atrium in a female patient presenting with valvular heart disease and no atrial fibrillation. The results of chest X-ray revealed a large opacity filling the lower right hemithorax, falsely interpreted as a mediastinal mass. During the tra...
متن کاملAortic valve homograft for revision surgery - transesophageal echocardiography considerations
Aortic root surgical anatomy and knowledge of the various homograft implantation techniques is of paramount importance to the attending anesthesiologist for echocardiographic correlation, estimation and accurately predicting aortic annular dimensions for the valve replacement in a case of diseased homograft.
متن کاملof 5 Echo for TEVAR : How Useful is TEE ?
Endovascular repair of the aorta (EVAR) has steadily gained popularity as a reliable alternative to conventional surgical repair of thoracic aortic aneurysms (TAA). The success of EVAR is critically dependent on demonstration of satisfactory graft deployment by various imaging modalities. Transesophageal echocardiography (TEE) is an ideal imaging tool for the thoracic and upper abdominal aorta ...

متن کاملEchocardiography guided therapy for massive intra-operative pulmonary emboli during arterio-venous fistula/graft thrombectomy.
Various techniques' are currently employed for thrombectomies of the arterio-venous (AV) fistula/graft to restore flow. Sub massive or massive pulmonary emboli's have been reported following such procedures both intra-operatively and post-operatively. The hemodynamic responses depend not only on the size of the emboli's but also on the pre existing cardiac or pulmonary reserve of the patient. R...
